Instructions for Updating the employment restriction on an Intern Credential
Intern credentials have employment restrictions, meaning they are only valid for use at the specific school district or charter network listed on the credential.
To make any changes to your Intern Credential, the California Commission on Teacher Credentialing (CTC) requires you (the candidate) to complete a Form 41-4 on their website. Once completed and signed, your application—along with any required documents—must be mailed by your Institution of Higher Education (Alternatives in Action & Reach).
Please note: All fees are set by and paid directly to the CTC and cannot be waived by Reach.
Step 1: Complete the Form 41-4 on the CTC Website
To avoid delays, follow the instructions below and complete each section accurately:
- Section 2 – Type of Application
Select the following:- Change of Employment Restriction (Application Fee: $50; Money Order cost $2.35 )
- Section 3 – Teaching Credential
Indicate the credential you are pursuing and leave the rest of the section blank:- Multiple Subject
- Single Subject
- Section 4 – Authorizations
Select the appropriate options:
- Single Subject Teachers:
- "English Learner Authorization"
- Specify your "Single Subject Teaching Area
- Multiple Subject Teachers:
- "English Learner Authorization"
- "Multiple Subject"
- Section 8 – New School Site Information
Enter your new school’s information. You can find the CDS code by Googling:"What is the CDS code for [School Name]?" - Section 9 – Oath and affidavit
Download, print, and sign the completed 41-4 form.
Step 2: Submit this Google Form
In the Google Form, you will be asked to upload your signed Form 41-4.
Step 3: Notify Your Advisor
After submitting the Google Form, inform your advisor. Once your advisor reviews your documents, they will contact you with the next steps, including instructions on how to pay the required CTC fee.
